*Chronic Disease Overview (CC3.a)

This course is designed to complement and provide Community Health Workers with chronic disease education, skill-building, and access to resources to prevent, manage, and improve self-management of chronic conditions.

Course Topics:
1) CHWs & Chronic Conditions Overview: Community Health Worker Online includes topics on the role of CHWs as a trusted member of the community, addressing barriers and cultural differences, the importance of chronic disease self-management and referring clients to self-management programs, communication skills, and working with members of the community and health care team.
2) High Blood Pressure & Hypertension: High Blood Pressure and Hypertension covers the facts about blood pressure, accurate blood pressure measurement, the importance of managing high blood pressure, including understanding and taking medication as prescribed, and how to help clients do self-measured blood pressure monitoring.
3) Prediabetes: Prediabetes covers the early signs and risks of developing diabetes, the lifestyle changes that can lower this risk, and how to support and encourage clients to participate in diabetes prevention programs.
4) Diabetes: Diabetes covers the facts and dangers of type two diabetes, daily management including checking glucose levels, and how to support self-management and referring patients to self-management programs.
5) Asthma: Asthma covers asthma facts, causes, diagnosis, monitoring asthma control, management including asthma medications, addressing environmental triggers, asthma action planning, and supporting children with asthma at home and school.
6) Breast Cancer: Breast cancer covers breast cancer, data and facts, risk factors, signs and symptoms, screening and diagnosis, and how CHWs can support clients in screening and prevention.
7) Cervical Cancer: Cervical cancer covers cervical cancer, data and facts, risk factors, signs and symptoms, screening procedures and recommendations, and how CHWs can support clients in preventing cervical cancer and seeking proper screening is.
8) Colorectal Cancer: Colorectal cancer covers what colorectal cancer, data is, and facts, risk factors, signs, and symptoms, recommended screening procedures and timelines, and how CHWs can support clients in preventing colorectal cancer and seeking proper screening.
9) Cholesterol Basics: Cholesterol Basics cover what cholesterol is and its purpose in the body, the difference between cholesterol the body makes and dietary cholesterol, the definitions and healthy ranges of total cholesterol, HDLs, LDLs, and triglycerides, myths and facts about cholesterol, what high cholesterol is, what causes it, and what its impact and burden are, estimating a client’s ASCVD risk using an online risk calculator, how high cholesterol is diagnosed and treated, modifiable and non-modifiable risk factors of high cholesterol, lifestyle changes that can affect cholesterol levels, and skills for helping clients prevent high cholesterol, and supporting clients that have high cholesterol.
10) This CHW COVID-19 module covers crucial topics including various roles CHWs can take in combating the spread of the coronavirus, how to talk with community members about COVID-19, disease spread and how to prepare and protect themselves, their family and their homes,common symptoms, testing options and how to safely connect clients.
