Description
Course Description: The Subjective, Objective, Assessment, and Plan (SOAP) note is an acronym representing a widely used method of documentation for Community Health Workers and other health care providers. The SOAP note is a way for CHWs to document their work with community members in a structured and organized way.

Learning objectives:

1) CHW students will learn how to use the SOAP documentation format
2) CHW students will learn to identify necessary components of professional writing and manage client records
3) CHW students will create a professional SOAP note
4) CHW students will learn how to apply HIPAA guidelines in their work to protect client’s information
